Job Summary
Conduct weekend Start of Care admissions for new home health patients, performing comprehensive skilled nursing assessments and developing individualized plans of care. Review medications, diagnoses, safety concerns, and caregiver needs while providing patient and family education. Complete accurate OASIS documentation and coordinate care with physicians and interdisciplinary team members. Communicate significant clinical findings and identify additional therapy needs. Requires active Minnesota RN license, CPR certification, previous home health experience, and strong OASIS knowledge. Compensation is $200 per visit plus mileage reimbursement.
Required Qualifications
- Active Minnesota Registered Nurse license
- Current CPR certification
- Previous home health nursing experience
- Strong OASIS knowledge
- Strong assessment and clinical decision-making abilities
- Excellent documentation and communication skills
- Valid driver's license, active insurance, and reliable transportation
Desired Qualifications
- Start of Care or admission experience strongly preferred
- Military medical experience
