Job Summary
Weekend Admission Nurse (RN) in home health caring for patients in designated territory. Responsibilities include start-of-care admissions (SOC) and OASIS assessments, follow-up visits, and conducting home visits to evaluate patient needs. Perform reconciliation of medications, wound care, treatments, and IV medication infusions per protocol. Monitor patient conditions, communicate changes to the clinical team, document observations and interventions, and collaborate with physicians and agency staff to deliver high-quality home health care. Eligible candidates will have CA RN license, BLS/CPR, CA driver’s license, and experience with SOC/OASIS; the role emphasizes autonomy, flexible weekend scheduling (including Saturdays/Sundays), and territory-based visits with mileage reimbursement and comprehensive benefits.
Required Qualifications
- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
- Previous experience with IV Medication infusions highly preferred
- Proficiencies initiating intravenous (IV) therapy is a plus
