Job Summary
The Waiver Case Manager provides comprehensive case management services for individuals receiving Minnesota waiver services, ensuring compliance with regulations, advocating for client needs, and maintaining accurate documentation. Key responsibilities include conducting intake interviews, developing individualized service plans, monitoring client services, maintaining case notes, and meeting billing compliance. Candidates should have a Bachelor’s degree in a related field and at least two years of experience working with vulnerable populations.
Required Qualifications
- Minimum two (2) years of full-time professional experience working with vulnerable populations.
- Strong organizational, communication, and documentation skills.
- Ability to manage deadlines, caseloads, and regulatory requirements.
Desired Qualifications
- Multilingual abilities (Spanish, Hmong, Burmese, Karen)
Additional Requirements
- Valid driver’s license and ability to meet MNCCS insurance requirements.
- Reliable transportation for required job duties.
- Daily local travel within assigned service areas.
