Job Summary
As a Waiver Case Manager, you will develop and monitor individualized care plans, coordinate services with interdisciplinary teams, and connect clients to community resources to enhance their independence. This role requires empathy, strong communication skills, and documentation proficiency. A Bachelor's degree in social work or related fields and a valid social work license are necessary, alongside relevant experience. Responsibilities include fostering client relationships, ensuring care compliance, and engaging in regular travel for client meetings.
Required Qualifications
- Bachelor’s degree in social work or related fields such as special education, psychology, nursing, human services.
- One year of experience in the education or treatment of people with developmental disabilities or minimum one course focusing on developmental disabilities.
- Valid driver's license and vehicle for work purposes.
Desired Qualifications
- Experience in CADI, BI, EW, DD waivers, and the AC program.
- Knowledge of MA, Medicare, and MN Healthcare programs.
- Familiarity with community resources and providers.
- Strong organizational skills and attention to detail.
- Proficiency in computer basics and familiarity with the Microsoft Office suite.
- Certified secondary language proficiency.
Additional Requirements
- Must be at least 18 years of age.
- Passing all background and requirements for facility and position.
- Ability to pass screening required by Federal Human Services Office of Inspector General.
