Job Summary
Provides nursing care as a Licensed Practical Nurse in a long-term care setting. Responsibilities include assessing residents on admission and ongoing, communicating significant changes to the IDT and families, documenting assessments, contributing to care plans, implementing nursing interventions per the care plan, safely administering medications and treatments (including IV therapy if certified), maintaining SBARR-based communications, monitoring resident status, supporting education for residents and families, and participating in QAPI-driven problem resolution. Requires current LPN license, CPR/AED certification, and IV certification (or willingness to obtain within six months). May require standing/walking for extended periods and ability to lift/move patients and equipment; continuing education commitments expected; familiarity with nursing documentation and interdisciplinary collaboration.
Required Qualifications
- Education from an accredited school of nursing resulting in becoming a Licensed Practical Nurse
- Current LPN license in the state you will be working (Oregon)
- CPR and AED certification required, or willing to obtain within six months of employment
- IV certification required, or willing to obtain within six months of employment
- Long-term care experience preferred
- Must be able to assess the basic nursing care needs of the geriatric resident
- Must possess excellent communication skills to interact with residents, visitors and staff
- Time management and organizational skills required
- Ability to stand and walk for long periods of time
- Ability to lift heavy objects using proper lifting techniques and with assistance when necessary
- Ability to transfer and move patients who vary with ability to assist weighing 200+lbs using proper lifting techniques and with assistance when necessary
- Ability to push and pull equipment weighing in excess of 40lbs using proper techniques and with assistance when necessary
- Manual dexterity, coordination and skillful use of hands when working with patients and equipment
- Visual acuity with the ability to distinguish color
- Ability to physically perform necessary documentation electronically and in writing
