Job Summary
Create and evaluate Individualized Service Plans with measurable vocational objectives for participants with intellectual and developmental disabilities. Provide ongoing case management, coaching, and direct supervision in office and community-based employment settings. Deliver training in job readiness and workplace skills while implementing accommodations to support successful outcomes. Coordinate intake assessments, track progress toward employment goals, and prepare monthly, quarterly, and annual reports. Maintain accurate case documentation, prepare unusual incident reports, and establish partnerships with employers, funding agencies, and community providers. Follow CARF and DDD regulatory standards while participating in quality assurance and program expansion initiatives.
Required Qualifications
- A bachelor's degree in Human Services, Special Education, Psychology, Social Work, or a related field
- At least one (1) year of supervisory experience working with individuals with intellectual or developmental disabilities
- A valid New Jersey driver's license in good standing with no more than five (5) points
- Successful completion of all required background screenings, including Criminal History, Central Registry, CARI, and applicable drug testing requirements
- Completion of required Division of Developmental Disabilities (DDD) pre-service trainings
- CPR/First Aid certifications
