Job Summary
Conduct home health visits to administer services, perform comprehensive assessments, and update physician orders. Manage patient treatment plans, coordinate care with the Clinical Team Manager and other team members, and initiate or revise plans of care as needed. Supervise Home Health Aides per regulatory requirements and handle re-certifications, transfers, and discharges. Document all activities in Synergy Software and communicate significant patient condition changes to the assigned team. Work 7 days a week with flexible scheduling, requiring availability for at least two-three days weekly, one weekend monthly, and two holidays annually. This role supports Alliance Healthcare's mission to provide high-quality, compliant care to patients in the Bakersfield, Lancaster, and So-Cal area.
Required Qualifications
- 1 year (Required)
- Nursing: 1 year (Required)
- Current CA RN License (Required)
- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Required)
