Job Summary
Conduct home health visits to administer services such as IVs, wound care, and assessments while maintaining patient independence. Perform comprehensive patient assessments, obtain physician orders, and document all interactions by uploading data to the Synergy Software portal. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise care plans, coordinating significant changes with physicians and other team members. Execute re-certifications, transfers, and discharges as requested, and supervise Home Health Aides per regulatory requirements. Maintain current CA RN licensure, BLS certification, and IV/Phlebotomy credentials. Work a flexible schedule of two to three days weekly with one weekend and holiday commitments.
Required Qualifications

- Nursing: 1 year (Required)
- Current CA RN License (Required)
- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Required)
