Job Summary
Facilitate patient care by visiting homes and long-term care facilities to evaluate patients, initiate telemedicine visits, and coordinate with providers. Review medical records, complete consents, and enroll eligible patients into Care Management plans. Capture diagnoses, conduct cognitive impairment and social determinants screenings, and reconcile medications. Provide post-discharge education, evaluate for adaptive equipment and safe environments, and appropriately document all Care Management activities. Full-time, salary-based role with 8-hour shifts Monday-Friday (8am-5pm); on-call may be required. Reliable transportation and a licensed driver's license are mandatory.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
