Job Summary
Facilitate receiving medical records, review documentation, and complete consents for patients in homes and long-term care facilities. Initiate Care Management plans, capture diagnoses, and coordinate care by reconciling medications and evaluating for home health, hospice, or RPM devices. Conduct cognitive impairment screenings, social determinants assessments, and post-discharge education while ensuring safe environments and adaptive equipment. Document all Care Management activities and collaborate with interdisciplinary teams to update care plans. Requires a licensed registered nurse with three years of clinical experience, reliable transportation for daily travel, and ability to work 8-hour shifts Monday through Friday. On-call rotation may be required.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
