Job Summary
Facilitate receiving medical records, review patient history, and complete consents for patients in homes, long-term care facilities, and ALFs. Initiate Care Management plans, coordinate care with providers, and reconcile medications while documenting activities and updating care plans. Perform cognitive impairment screenings, SDoH assessments, and ACP reviews; evaluate for home health, hospice, RPM devices, and adaptive equipment. Provide post-discharge education and ensure safe environments. Requires reliable transportation for daily travel between service areas including Hepzibah, Waynesboro, and Thompson. Full-time, salary-based 8-hour shifts Monday-Friday with on-call availability.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
