Job Summary
Registered Nurse to service patients in the Rock Hill area as a Visiting Nurse/Nurse Case Manager. The role involves visiting patients in their homes and long-term care facilities to evaluate patients and initiate telemedicine visits, delivering quality healthcare in compliance with applicable laws and policies. Responsibilities include receiving and reviewing medical records, enrolling patients in Care Management, initiating Care Plans of Care, documenting diagnoses, coordinating with care teams, performing AWVs and cognitive/social screenings, evaluating for home health/palliative/consults, and ensuring accurate documentation. Travel to patient homes and facilities is required on a daily basis; on-call may be required. Qualifications emphasize active nursing licensure, RN preferred, good standing, and relevant clinical/community experience, with strong communication and teamwork skills.
Required Qualifications
- Must be a licensed nurse
- Registered nurse preferred
- License in good standing with appropriate board/issuer
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Strong written and verbal skills
- Ability to manage and demonstrate effective leadership skills
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order
