Job Summary
Facilitate receiving medical records, review patient files, and complete consents for eligible patients. Initiate Care Management plans, capture diagnoses with highest specificity, and perform cognitive impairment and Social Determinants of Health screenings. Coordinate care by reconciling medications, providing post-discharge education, and evaluating needs for home health, hospice, or adaptive equipment. Document all Care Management activities and collaborate with the interdisciplinary team to update care plans. Travel daily to patient homes, long-term care facilities, and initiate telemedicine visits within the West Ashley service area. Full-time, salary-based role with 8-hour shifts Monday-Friday (8am-5pm); on-call rotation may be required.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
