Visiting Nurse

Job Summary
Facilitate patient care by visiting homes, long-term care facilities, and initiating telemedicine visits within the Mt. Pleasant service area. Review medical records, complete consents, and enroll eligible patients into Care Management plans. Execute comprehensive assessments including cognitive screening, Social Determinants of Health, and Advance Care Planning. Coordinate care by reconciling medications, evaluating for home health or hospice needs, and providing post-discharge education. Document all Care Management activities and maintain up-to-date care plans. Requires a licensed Registered Nurse with at least three years of clinical experience, reliable transportation, and ability to work 8-hour shifts Monday through Friday. On-call rotation may be required.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
