Visiting Nurse- RN

Job Summary
Conduct home and facility visits to evaluate patients, initiate telemedicine, and coordinate care with providers. Facilitate medical record transfers, complete consents, and enroll eligible patients into Care Management plans. Assess cognitive impairment, social determinants of health, and acute conditions while reconciling medications and evaluating for adaptive equipment or RPM devices. Document Care Management activities, provide post-discharge education, and collaborate with interdisciplinary teams to update care plans. Travel daily to Orangeburg area homes, long-term care facilities, and ALFs/ILFs with reliable transportation. Full-time, salary-based 8-hour shifts Monday through Friday; on-call rotation may be required.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
