Visiting Nurse- RN

Job Summary
Facilitate patient visits to homes, long-term care facilities, and ALFs to evaluate conditions, initiate telemedicine, and coordinate care with providers. Review medical records, complete consents, and create care management plans including cognitive screenings, SDoH assessments, and medication reconciliations. Provide post-discharge education, evaluate for adaptive equipment and RPM devices, and document all Care Management activities. Travel daily between service areas 29018, 29030, 29039, and 29047, requiring reliable transportation and a licensed driver's license. Work full-time 8-hour shifts Monday through Friday with possible on-call requirements.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
- On call maybe required
