Job Summary
Facilitate receiving medical records, complete consents, and enroll eligible patients into Care Management while initiating comprehensive Care Management Plans. Evaluate patients in homes and long-term care facilities to capture diagnoses, screen for cognitive impairment and social determinants of health, and assess eligibility for home health, hospice, or RPM devices. Reconcile medications, provide post-discharge education, and coordinate care with interdisciplinary teams to ensure safe environments and up-to-date care plans. Complete AWVs, cognitive screenings, and SDoH assessments as required. Full-time, salary-based 8-hour shifts Monday through Friday (8am-5pm) with possible on-call rotation. Reliable transportation and a licensed driver's license are mandatory for daily travel.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
- Must be available for on call
Desired Qualifications
- Registered nurse
- A minimum of three (3) years clinical experience
- Experience in community settings
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
