Job Summary
Visiting Nurse – Registered Nurse; provides in-home and facility-based visits within Gainesville area; travel required daily; telemedicine initiation; 8-hour shifts Mon-Fri 8am-5pm; performs care coordination, reviews records, enrolls patients in Care Management, completes consents and assessments, documents care plans, coordinates with providers and care team; requires licensed nurse with RN preferred; 3+ years experience preferred; willing to travel between offices and facilities; valid driver's license and insured vehicle.
Required Qualifications
- Must be a licensed nurse.
- Registered nurse preferred.
- License must be in good standing with appropriate board/issuer.
- A minimum of three (3) years clinical experience preferred.
- Experience in community settings preferred.
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order
