Visiting Nurse - Registered Nurse

Job Summary
Facilitate patient care by visiting homes, long-term care facilities, and initiating telemedicine visits to evaluate patients and coordinate with providers. Review medical records, complete consents, and enroll eligible patients into Care Management plans while capturing diagnoses and assessing social determinants of health. Reconcile medications, provide post-discharge education, and evaluate for adaptive equipment or home health services. Document all Care Management activities and communicate with interdisciplinary teams to keep care plans current. Requires reliable transportation for daily travel between Tifton and Albany service areas, with on-call availability. Full-time 8-hour shifts Monday through Friday.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
