Job Summary
Facilitate receiving medical records, complete consents, and enroll eligible patients into Care Management. Initiate care plans, capture diagnoses, and perform cognitive impairment and social determinants of health screenings. Evaluate patients for home health, hospice, RPM devices, and adaptive equipment while coordinating care and reconciling medications. Provide post-discharge education, document activities, and update care plans in conjunction with the care team. Travel daily to patient homes and long-term care facilities in the Augusta area for visits and telemedicine. Full-time, salary-based role with 8-hour shifts Monday through Friday.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
