Visiting Nurse- Clinic

Visiting Nurse (Nurse Case Manager) to service patients in the Chapin Clinic area, traveling daily to patients’ homes and facilities to evaluate and coordinate care, initiate telemedicine visits, and enroll patients in Care Management Plans. Responsibilities include reviewing medical records, obtaining consents, enrolling eligible patients, coordinating with the care team, documenting activities, and providing post-discharge education. Must be a licensed nurse with good standing; RN preferred; experience with home health, IVs/phlebotomy, and community settings is a plus. Position is full-time, 8-hour shifts, Monday–Friday (8am–5pm), with travel between offices, facilities, and patient homes; driver’s license and insured vehicle required.
