Visiting Nurse- Clinic

Job Summary
Facilitate patient visits in homes and long-term care facilities within the Chapin Clinic area, evaluating patients, initiating telemedicine, and coordinating care with providers. Review medical records, complete consents, enroll eligible patients in Care Management, and create comprehensive plans of care. Assess cognitive impairment, social determinants of health, and acute conditions; evaluate for home health, hospice, RPM devices, and adaptive equipment. Reconcile medications, provide post-discharge education, and accurately document all Care Management activities. Requires a licensed Registered Nurse with three years clinical experience, reliable transportation for daily travel, and ability to work 8-hour shifts Monday through Friday. On-call rotation may be required.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
- Must have reliable transportation
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in aesthetics
- Experience with IV's
- Experience with phlebotomy
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities
- Ability to demonstrate sound judgment
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
