Job Summary
Conduct home health visits to administer IVs, change dressings, clean wounds, and perform comprehensive patient assessments. Document all findings and upload data to the Synergy Software portal while obtaining and verifying physician orders. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise patient care plans, coordinating significant condition changes with physicians and other team members. Execute re-certifications, transfers, and discharges as directed, including supervising Home Health Aides per regulatory requirements. Maintain current CA RN licensure, BLS certification, and IV/Phlebotomy credentials. Work a flexible schedule of two to three days weekly with on-call rotations for weekends and holidays.
Required Qualifications

- Nursing: 1 year (Required)
- Current CA RN License (Required)
- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Required)
