Job Summary
Visit RN - Home Care & Hospice. 0.6 FTE (24 hours/week) on 2nd shift (1:30-10pm), 3 weekdays with no weekends or on-call; coordinates and oversees the interdisciplinary plan of care for patients in home health and home hospice environments; works with patients, families, nurses, social workers, physicians, and community agencies; care for an average of 4-6 patients per shift; requires strong interpersonal, negotiation, analytical and computer skills; EPIC experience preferred; CA #LI-MO/CA #LI-TG; prior home health experience is beneficial.
Required Qualifications
- Associate’s Degree in Nursing required
- Current State of WI or eNLC compact state RN license
- Valid State of WI driver’s license with an acceptable driving record
- Current BLS (CPR) certification or ability to obtain during orientation
