Visit Registered Nurse - PRN

Perform focused nursing assessments, address acute patient needs, and administer prescribed medications while educating families on care techniques. Develop and revise individualized care plans, counsel patients on symptom management, and maintain accurate clinical documentation. Respond promptly to condition changes, assist with activities of daily living, and collaborate with case managers to ensure timely supplies and services. Participate in interdisciplinary team meetings and quality improvement initiatives. This role requires an RN licensed in the state of employment with at least one year of hospice experience, plus BLS certification and a valid driver's license. Mileage reimbursement is provided for home visits.
