Visit Registered Nurse - PRN

PRN Visit RN provides in-home hospice nursing care, performing focused nursing assessments, administering prescribed medications and treatments, educating families on care techniques, developing and revising individualized care plans with patients and caregivers, maintaining accurate clinical documentation, and collaborating with the interdisciplinary team to ensure timely needs are met and supplies/services are provided. Responsibilities include symptom management, end-of-life care, assisting with activities of daily living, responding to changes in patient condition, and participating in quality improvement initiatives; requires strong communication, self-motivation, and ability to work with minimal supervision.
