Visit Registered Nurse - PRN

Perform focused nursing assessments, administer prescribed medications, and counsel families on symptom management for hospice patients in their homes. Develop and revise individualized care plans while maintaining accurate clinical documentation and assisting with activities of daily living. Respond promptly to changes in patient condition and collaborate with case managers to ensure timely supplies and services. Participate in interdisciplinary team meetings to ensure continuity of care. Requires RN licensure, CHPN certification preferred, and minimum one year of hospice experience. Mileage reimbursement and tools provided for home visits.
