Visit Registered Nurse - PRN

Perform focused nursing assessments, administer prescribed medications and treatments, and counsel patients and families on symptom management and end-of-life issues in home visits. Develop and revise individualized care plans, maintain accurate clinical documentation, and respond promptly to changes in patient condition or caregiver concerns. Collaborate with case managers to ensure needed supplies and services are provided while participating in interdisciplinary team meetings and quality improvement initiatives. Ensure continuity of care through effective communication with the interdisciplinary team. This role requires an RN licensed in the state of employment with at least one year of hospice nursing experience. Mileage reimbursement and tools are provided for home visits.
