Visit Registered Nurse - PRN

Perform focused nursing assessments, administer medications, and address acute needs of hospice patients in their homes. Develop and revise individualized care plans, counsel families on symptom management, and maintain accurate clinical documentation. Ensure continuity of care through communication with the interdisciplinary team and respond promptly to changes in patient condition. Collaborate with case managers to provide needed supplies and services while participating in quality improvement initiatives. Requires RN licensure, CHPN certification preferred, and minimum one year hospice experience. Mileage reimbursement and tools provided for home visits.
