Visit Registered Nurse - PRN

Conduct focused nursing assessments and administer prescribed medications to hospice patients in their homes. Develop and revise individualized care plans while counseling families on symptom management and end-of-life issues. Ensure continuity of care through effective communication with the interdisciplinary team and maintain accurate clinical documentation. Respond promptly to changes in patient condition or caregiver concerns and assist with activities of daily living. Participate in interdisciplinary team meetings and quality improvement initiatives. Requires RN licensure, CHPN certification preferred, and minimum one year of hospice experience. Mileage reimbursement and tools provided for home visits.
