Job Summary
Review your case load, prepare technology, and navigate your schedule to partner with clinical in-home team members for virtual wellness visits. Conduct thorough health risk assessments, medication reviews, cognitive screenings, and educational sessions with diverse patients. Collaborate directly with Sprinters to evaluate vital signs, arrange blood draws, and manage referrals, preventive care interventions, and community resources. Document findings and provide comprehensive care coordination while maintaining HIPAA secure messaging protocols. Requires a Board Certified Family Nurse Practitioner with an active California license and Medi-Cal credentialed status. Full-time remote role with secondary locations in California.
Required Qualifications
- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner License in California
- Credentialed Medicaid provider for the state of California (Medi-Cal)
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
