Job Summary
Review your daily case load, prepare technology, and navigate your schedule to partner with clinical in-home team members for virtual wellness visits. Conduct thorough health risk assessments, medication reviews, and cognitive screenings while making warm connections with diverse patients. Collaborate directly with Sprinters to evaluate vital signs, arrange blood draws, and carry out necessary tasks to address specific patient needs. Provide comprehensive care coordination and management, including preventive care interventions, medication management, referrals to specialists, and documentation of findings. Board certified Family Nurse Practitioner with an active license in KY, MI, OH, or IL required.
Required Qualifications
- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner license in any of the following states: KY, MI, OH, IL
- Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
