Job Summary
Review your daily case load and prepare technology to service patients at their homes. Conduct virtual wellness visits, health assessments, and cognitive screenings while collaborating with in-home clinical team members to evaluate vital signs and arrange blood draws. Perform thorough health risk assessments, medication reviews, and preventive care interventions, followed by comprehensive care coordination including referrals and documentation. Maintain high-quality standards through autonomous work using secure messaging apps and laptops for charting. This role offers a 3-week paid sabbatical in Q1 for qualified candidates.
Required Qualifications
- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner License in South Carolina
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
