Job Summary
Conduct tele-health visits, telephonic triage, and email follow-ups for One Medical patients via video appointments and phone calls. Engage in weekly Clinical Rounds and participate in daily huddles to collaborate with virtual and in-office teammates. Deliver evidence-based primary care to patients of all ages using advanced technology while maintaining continuous clinical learning. This full-time role requires NY licensure and weekend availability. Required Qualifications
- Completed an accredited FNP program with a national certification
- In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an outpatient primary care setting seeing patients of all ages (0+)
- In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+)
- Currently licensed in NY with ability to obtain additional state licenses
- Ability to work weekday and weekend shifts (every Saturday or Sunday required)
- Excellent clinical and communication skills
- Clinical proficiency in evidence-based primary care
- The ability to confidently navigate uncertain situations with both patients and colleagues
- Readiness to adapt personal and interpersonal behavior to meet the needs of our patients
- Authorized to work in the U.S. (E-Verify)
Desired Qualifications
- Currently licensed in NY with ability to obtain additional state licenses (if not currently licensed)
