Job Summary
Conduct tele-health visits, including video appointments, telephonic triage, and email follow-ups, while collaborating with virtual and in-office teammates via daily huddles. Engage in weekly Clinical Rounds to support continuous learning and maintain clinical proficiency in evidence-based primary care. Manage patient care across all ages through 24/7 coverage, utilizing advanced technology to deliver high-quality, human-centered care. Requires New York licensure, FNP certification, and availability for weekend shifts. Full-time role with robust benefits, including malpractice insurance, CME stipends, and paid sabbatical.
Required Qualifications
- Completed an accredited FNP program with a national certification
- In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an outpatient primary care setting seeing patients of all ages (0+)
- In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+)
- Currently licensed in NY with ability to obtain additional state licenses
- Ability to work weekday and weekend shifts (every Saturday or Sunday required)
- Excellent clinical and communication skills
- Clinical proficiency in evidence-based primary care
- The ability to confidently navigate uncertain situations with both patients and colleagues
- Readiness to adapt personal and interpersonal behavior to meet the needs of our patients
- Authorized to work in the U.S. (E-Verify)
