Job Summary
Conduct tele-health visits, telephonic triage calls, and email follow-ups to treat patients virtually alongside primary care providers. Participate in weekly Clinical Rounds and daily huddles to collaborate with both virtual and in-office teammates. Utilize clinical training to deliver evidence-based care while navigating uncertain situations with patients and colleagues. Requires NY licensure, FNP certification, and ability to work weekday and weekend shifts including Saturdays and Sundays. Full-time 40-hour schedule with evening and weekend differentials.
Required Qualifications
- Completed an accredited FNP program with a national certification
- In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an outpatient primary care setting seeing patients of all ages (0+)
- In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+)
- Currently licensed in NY with ability to obtain additional state licenses
- Ability to work weekday and weekend shifts (every Saturday or Sunday required)
- Excellent clinical and communication skills
