Job Summary
Conduct tele-health visits, including video appointments, telephonic triage calls, and email follow-ups for One Medical patients. Participate in weekly Clinical Rounds and collaborate with virtual and in-office teammates via daily huddles to provide evidence-based primary care. Deliver high-quality care across all ages using advanced technology while maintaining continuous learning through clinical modalities. Work full-time 40-hour shifts including evenings and weekends, with rotations ranging from 6am to 12am EST.
Required Qualifications
- Completed an accredited FNP program with a national certification
- In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an outpatient primary care setting seeing patients of all ages (0+)
- In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+)
- Currently licensed in NY with ability to obtain additional state licenses
- Ability to work weekday and weekend shifts (every Saturday or Sunday required)
- Excellent clinical and communication skills
