Job Summary
Review your case load, prepare technology, and navigate your daily schedule to partner with in-home clinical team members. Conduct virtual adult and pediatric wellness visits, health assessments, and proactive care including thorough health risk assessments, medication reviews, and cognitive screenings. Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and perform necessary tasks to address specific patient needs. Provide comprehensive care coordination and management, including preventive care interventions, medication management, referrals to specialists, and documentation of findings. Ensure high-quality service through strong communication, independent problem-solving, and adherence to HIPAA secure messaging protocols.
Required Qualifications
- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner License in New York
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
