Job Summary
Conduct 30-minute video visits with Medicare, Medicaid, and commercial members, reviewing medication history and educating patients on their health. Maintain a 40-hour weekly schedule with consistent availability for 320+ monthly capacity units, executing a visit completion rate of 220+ visits per month. Work in a private space with broadband connectivity to uphold member privacy and meet all training and compliance requirements. Maintain active state licenses in coverage areas including Oregon and engage regularly with management and clinical teams.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Multi-state licenses (preferred)
- Broadband connectivity with video and audio enabled
- A quiet setting with a neutral background from which to conduct visits
- Ability to uphold health plan member privacy per CVS Health guidelines
- Ability to view and read a computer screen, iPad, and other electronic devices
- Effective verbal, written, and electronic communication skills
- Ability to work without direct supervision and practice autonomously
- Proficiency with information management and technology
- 40 hours of availability weekly
- Consistently pacing 220+ completed visits per month
- Execute a strong visit completion rate
- Regular training required
- Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
- Maintain active state licenses in coverage areas
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Must meet minimum requirements for remote visit delivery
- Consistently engage with regular manager, team and clinical meetings
- Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
Desired Qualifications
- Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format
