Job Summary
Conduct 30-minute Video Visits with Medicare Advantage, Medicaid, and commercial plan members, performing medication and medical history reviews while educating members and answering questions. Maintain a visit completion rate of 220+ visits per month and provide 320+ capacity units monthly within a private space with adequate broadband. Execute training and compliance with Signify Health Video Visit requirements, consistently engaging in regular manager, team, and clinical meetings. Account for operational, clinical, and member experience key performance indicators and metrics. Maintain active state licenses in coverage areas, including Missouri, and uphold health plan member privacy per CVS Health guidelines.
Required Qualifications
- Board certification
- Active, unrestricted license(s) in coverage area(s)
- Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs)
- Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Proficiency with information management and technology
- Ability to work without direct supervision and practice autonomously
- Effective verbal, written, and electronic communication skills
- Specific vision abilities include the ability to view and read a computer screen, iPad, and other electronic devices
- Must meet minimum requirements for remote visit delivery, including broadband connectivity with video and audio enabled
- A quiet setting with a neutral background from which to conduct visits
- The ability to uphold health plan member privacy per CVS Health guidelines
- Consistently engage with regular manager, team and clinical meetings
- Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
Desired Qualifications
- Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format (preferred)
- AGNP certification preferred for NPs
