Job Summary
Conduct comprehensive virtual onboarding, annual, and follow-up visits for complex chronic conditions while collaborating with in-home Health Advocates. Manage a centralized inbox queue by signing off normal results, handling urgent clinical triage, and routing complex issues during provider absences. Execute thorough clinical handoffs back to the primary longitudinal care pod and support preventative-care quality campaigns including HEDIS/STARs gap closure. Maintain accurate EMR documentation with rigorous diagnostic coding and adhere to evidence-based care plans. This part-time role requires a flexible schedule with month-ahead coverage planning, dedicated inbox windows, and optional same-day flex windows. Candidates must hold an active NP or PA license in Michigan, Massachusetts, or Colorado, along with 3+ years of adult primary care experience.
Required Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license
- Active NP/PA licensure in at least one of the following core markets: MI, MA, or CO (with a willingness to obtain additional state licenses as requested)
- Active DEA registration and state controlled-substance license
- Current BLS certification
- 3+ years of experience in adult primary care, family medicine, urgent care, or emergency medicine
Desired Qualifications
- Prior high-volume virtual/telehealth care experience
- Working knowledge of value-based care, HEDIS/STARs, and clinical documentation improvement (CDI)
- Hands-on experience using AI tools in a clinical setting to optimize efficiency
