Job Summary
Conduct physical exams, order and interpret diagnostic tests, and develop treatment plans for acute and chronic illnesses in older adults under physician supervision. Manage patient care for conditions including diabetes, COPD, and CHF while ensuring compliance with CLIA, HIPAA, and accrediting agency standards. Document patient visits and health care plans in the EMR within 48 hours and coordinate care with hospitalists when patients are admitted. Maintain patient confidentiality, handle emergency situations calmly, and provide education to promote health and reduce risk factors. Ensure patients are seen every other visit by a physician and complete all operational tasks per departmental guidelines.
Required Qualifications
- Graduate of an accredited school of nursing
- Certified as a nurse practitioner with current state licensure
- At least 1 year of previous professional nursing experience with 1-3 years experience as a practicing as a primary care nurse practitioner
- Prior experience working in an electronic medical records system
- ACLS/BLS certification required as mandated by state
- Must demonstrate knowledge of the rationale of appropriate patient care
- Demonstrate ability to handle emergency situations calmly and effectively
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to effectively present information and respond to questions from a various group of people
- Ability to define problems, collect data, establish facts, and draw valid conclusions
- Excellent written and verbal communication, teaching ability, and good telephone skills
- Working knowledge with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration
- Full range of body motion, including handling and transferring patients, manual and finger dexterity; including eye-hand coordination
- Requires standing, walking, pushing, bending, kneeling, and reaching at arm's length and overhead in a clinic for prolonged periods of time
- Must be able to lift and move patients and medical equipment as needed
- Requires ability to lift/carry up to 15 lbs
- Requires corrected vision and hearing to normal range
- Ability to sit for extended periods of time
- Ability to operate a motor vehicle and have own means of transportation
