Job Summary
Senior executive role leading all aspects of Case Management, including strategy development, utilization management, coordination of care, and transition management. Responsible for aligning Case Management initiatives with regulatory requirements and organizational goals, ensuring performance metrics are met, and maintaining HIPAA-compliant handling of PHI. Leads cross-department collaboration to integrate Case Management/Social Work services, oversees quality of care, policy development, and regulatory readiness; requires extensive nursing leadership experience, solid knowledge of CMS and HIPAA regulations, and demonstrated strategic and operational leadership in acute care settings.
Required Qualifications
- Bachelor’s Degree in Nursing
- New York State Licensed Registered Professional Nurse
- Certification in Patient Review Instrument (PRI)
- Certification in Case Management
- Certification in Hospital Management
- Strong knowledge of CMS Conditions of Participation
- Proficiency in Microsoft Office Suite
- Excellent written and verbal communication skills
- Strong interpersonal and leadership skills
