Job Summary
Assess needs, develop client-specific service plans, and coordinate services for veterans, caregivers, and family members across ETCOG's 14-county region. Monitor services and spending, maintain accurate case records, and communicate with VA partners and service providers to ensure compliance and program accountability. Educate participants on program benefits and responsibilities while identifying and resolving service gaps or risks to support veteran independence. Requires high school diploma or equivalent, preferred bachelor's degree in human services or related field, and minimum one year of experience in case management or veteran services. High school diploma or equivalent required. Graduation from an accredited four-year college or university with major coursework in human services, social science, business, or a related field is preferred. Minimum one year's experience in human services, case management, veteran services, long-term services and support, aging services, disability services, or a closely related field required; experience working with veterans, older adults, individuals with disabilities, caregivers, and community-based service systems is strongly preferred. Veteran status is preferred due to the position's direct service to veterans and veteran caregivers. Veterans are strongly encouraged to apply but veteran status is not required for consideration. Equivalent combinations of education and relevant experience may be considered.
