Veteran's Housing Integrated Care Specialist

Job Summary

Maintain housing stability for veteran households by conducting daily intake assessments, coaching on activities of daily living, and managing crisis interventions. Develop individualized recovery plans and progress reports, ensuring five documented contacts per day and accurate HMIS data entry. Screen for eligibility, advocate for financial assistance and landlord negotiations, and coordinate with community partners to secure permanent housing. Assist veterans with entitlement applications for SSI/SSDI and medical cards while building rapport and promoting self-sufficiency. Submit case files for supervision review within designated timeframes and attend required training and meetings.

Required Qualifications

  • Bachelor's degree in a human service field
  • Four years of equivalent education and/or experience working in the human services field
  • Computer literacy
  • Valid Kansas driver's license
  • Access to personal vehicle
  • Ability to write and communicate verbally in a clear and concise fashion
  • Ability to develop and maintain rapport with veterans, constituents and staff
  • Knowledge of community-based and Veteran-serving resources
  • Experience working in the community with veterans in the area of homelessness
  • Experience working in the community with veterans in the area of substance abuse
  • Experience working in the community with veterans in the area of family services
  • Experience working in the community with veterans in the area of mental health
  • Experience working in the community with veterans in the area of PTSD
  • Experience working in the community with veterans in the area of crisis intervention
  • Experience working in the community with veterans in the area of incarceration
  • Ability to lift 30 pounds
  • Ability to bend
  • Ability to stoop
  • Ability to climb stairs
  • Maintain acceptable overall attendance record
  • Flexibility in work schedule when needed
  • Ensure safe transportation of residents in the community

Desired Qualifications

  • Licensed Master's degree
