Job Summary
The VDC Person-Centered Counselor coordinates, administers, and provides supervisory and administrative support for the VDC program between Senior Resource Alliance, the VA, eligible veteran beneficiaries and/or their representatives, and fiscal staff. Responsibilities include program orientation on self-direction, assisting veterans/representatives with planning, service provision, and budget management to enable independent living in the community, and training in self-direction. Conducts virtual or home-based visits to assess needs, develop service plans, and reassess as conditions change; assists with monthly invoices and interacts with fiscal staff on setup, budget, and invoicing. Collaborates with VA Medical Center liaisons and fiscal staff to enroll veterans, communicates ongoing issues, maintains current veteran caseloads and budgeting records, and ensures timely submission of reports. Maintains client data confidentiality in line with federal/state regulations, attends conferences to stay updated, and participates in disaster operations when required. The role involves keeping up-to-date on issues affecting veterans and serving as a resource for organizations and the public.
