Lead care coordination for residents with dementia, physical limitations, and palliative care at Amsta's De Keyzer location. Regulate care plans, coach colleagues on methodical work, and translate improvement opportunities into concrete team actions. Collaborate closely with treating professionals, family members, and other disciplines to ensure personalized care and early identification of care needs. Drive process improvements while maintaining a warm, small-scale environment where residents determine their daily routines.
Required Qualifications
- Een afgeronde hbo-opleiding verpleegkunde
- Een geldige BIG-registratie
- Ervaring in de ouderenzorg of langdurige zorg
- Kennis van kwaliteitsverbetering en methodisch werken
- Het aanleveren van een Verklaring Omtrent Gedrag (VOG)
