Support and guide clients in their daily lives, assisting with personal care while encouraging self-management and autonomy. Observe client behavior and development to signal changes in well-being, then report these updates to the multidisciplinary team. Collaborate with nurses and other disciplines to contribute to treatment plan execution and actively participate in team meetings to share observational insights. Motivate clients for daily functioning and provide support for activities of daily living and daily structure. Engage in ongoing discussions regarding improvements to care and the living environment on the ward.
