Job Summary
Coordinate day and evening care on the Laarstaete site in Lelystad, ensuring all services are aligned with stakeholders. Identify bottlenecks in both planned and unplanned care to propose improvements and signal these to colleagues for optimization. Contribute to quality enhancement initiatives and support personal development for the team. Requires a completed Level 4 nursing education, BIG registration, and experience coordinating care across somatic, geriatric, rehabilitation, and recovery groups. Schedule includes 24 hours per week with on-site availability.
Required Qualifications
- Afgeronde opleiding Verpleegkundige niveau 4
- Geldige BIG registratie
- Kennis en vaardigheden van de verschillende doelgroepen zoals somatiek, pg, revalidatie en herstel
- Ervaring met het coördineren van zorg
- Beschikbaarheid voor de verpleegkundige diensten op de locatie
- Geldige VOG
