Job Summary
Coordinate daily care for 24 young adult clients with serious psychiatric conditions, including substance use issues, while supporting their transition to independent or supervised housing. Observe mental, physical, and social functioning to inform personalized recovery plans and maintain close contact with family members and external stakeholders. Execute treatment plans, draft guidance and signaling plans, and ensure accurate documentation and reporting. Motivate and guide clients through stabilization and insight-building while contributing to team professional development and quality improvement. Supervise students and interns within the multidisciplinary team.
Required Qualifications
- MBO of HBO BIG geregistreerde verpleegkundige
